OK, let's see. There's medicine. There's inflating tires - hypodermics are a kind of inflationary device. There's drugs as a rite of passage and a common childhood thing, if childhood extends through adolescence. Change a tire is metaphorical for learning a trade. Learning a blue-collar trade, changing tires, fixing cars is the opposite of, say, learning a high-paying profession like medicine.
